How to Request Medical Records

To obtain your health records, please follow these steps:

• Submit a written application at the hospital reception or records department
• Provide valid ID proof (Aadhaar Card, PAN Card, etc.)
• Mention details such as patient name, date of admission, and type of records required

Processing Time:

Requests are generally processed within 3–7 working days, depending on the nature of the request.

Note: In some cases, nominal administrative charges may apply for copies of records.

Admission, Discharge & Medical Records

We provide the following documents to patients:

• Admission details
• Discharge summary
• Prescription records
• Diagnostic reports (if conducted at our hospital)

These documents are important for future treatment and medical reference.

Birth Records

After delivery, the hospital provides:

• Birth report / discharge summary

This document can be used to apply for an official Birth Certificate from the local Municipal Authority.

We recommend registering the birth within the time period prescribed by government rules.

Death Records

In case of a patient’s demise, the hospital provides:

• Medical Certificate of Cause of Death (MCCD)

The official Death Certificate is issued by the local government authority. Our staff will guide the family through the process if needed.

Correction of Medical Records

If you believe that any information in your medical record is incorrect or incomplete, you may request a correction.

• Submit a written request with proper details
• Attach supporting documents (if required)

Our team will review your request and respond within a reasonable timeframe.
